What do we do?

Age Connects Cardiff & the Vale Discharge Support Officer (DSO) Service work within the Integrated Discharge Service (IDS) in Cardiff and the Vale of Glamorgan to assist older people over the age of 60, who are in hospital, and their families with discharge planning. The posts are aligned to specific wards across the University Hospital Wales and University Hospital Llandough, as well as St David’s and Barry hospitals The team work with other disciplines in the IDS including Social Workers, First Point of Contact Officers, Discharge Liaison Nurses to ‘enhance visibility on key wards, provide an effective interface/”progress chasing” function between ward areas and the IDS and to provide better discharge planning advice/support across simple as well as complex discharges’. 

Referrals are generated primarily through the IDS and the wards.

The Age Connects Cardiff & the Vale Broker Service works proactively within the locality-based Community Resource Teams C1 Vale Contact Centre and Liaison Psychiatry for Older People Team based in University Hospital Wales and University Hospital Llandough. The aim of the service is to maximise patient independence by providing rehabilitative support and information to older people and their families in their own homes: 
 

  • On their discharge from hospital 
  • To prevent avoidable hospital admissions 
  • To help chose an appropriate care home 


We work in partnership to ensure that a range of voluntary sector health and social care services are pulled together in response to the identified needs of older people who have been referred to the Broker Service. This provides a seamless transition from hospital to community, ensuring that information on Third Sector services is readily available to all relevant professionals and to pull together service provision to meet identified need. 

Referrals can only be taken through the Cardiff CRTs, CIV and the LPOP Team
